國立陽明交通大學圖書館借閱證申請服務
國立陽明交通大學圖書館基於「申請借閱證」之目的，須蒐集您的「姓名、E-mail、連絡電話」等個人資料，以在校務行政期間及地區內，作為借閱證申請及必要聯繫之用。
依個人資料保護法，您得就其個人資料行使請求查詢、閱覽、製給複製本、補充或更正、停止蒐集、處理或利用、刪除等權利。
各項資料如未完整提供，恐將無法進行借閱證申請相關作業。

National Yang Ming Chiao Tung University Library for the purposes of collections and safety maintenance, shall request applicant’s representative’s information on the name, E-mail and contact phone number. Such information, within the location and period of school administration, shall be used to create an account, library management, conduct analysis and make necessary correspondence.
According to the Personal Data Protection Act, you may exercise the following rights by contacting us:
- (1)inquiry and request for a review of the aforementioned personal information;
- (2)request to make duplications of the aforementioned personal information;
- (3)request to supplement or correct the aforementioned personal information;
- (4)request to discontinue collection, processing or use of aforementioned personal information; and
- (5)request to delete the aforementioned personal information.
The application must be filled out completely and required documentation must be provided at the time of the application is submitted. Incomplete applications will not be accepted or processed.
